Rank One Sports Electronic Forms

Parent information

From a computer or IPad, go to www.rankone.com (if using a mobile device use the regular web address not the mobile site for rank 1.)

At the top right corner of the page choose the option for Fill Out Electronic Forms.

1. Select your state from the drop down box (Texas)

2. Select Mount Pleasant ISD from the list

You will need your students ID number from MPISD, your student will know this number.

3. Hover your mouse over the Electronic Participation Forms Tab. A list of required forms will be listed in red, click on the first form.

4. Enter your student’s information (First Name, Last Name, and ID Number)

5. Complete all fields on the form.

6. When prompted use the mouse to sign your name in the box, if using a mobile device use your finger or stylus.

7. Click the I agree box for electronic signature purposes.

8. Submit form

9. Click on the “Next Form” button or Return to the list and choose your next form

10. Your child information should automatically be inserted into the correct places at this point.

11. Continue answering questions and signing documents.

12. Once completed and the Athletic Training Staff approves the forms you will receive an email notification that your students forms were accepted.
